Improving Performance with Knowledge
The Indiana Primary Health Care Association (IPHCA) and the Indiana Quality Improvement Network, a health center-controlled network (IQIN HCCN), have partnered to streamline quality improvement training efforts for community health centers.
Both IPHCA and IQIN share a goal to improve the performance of clinical and operational quality measures using data, health IT, and implementation of promising practices around Indiana.
Focus on Quality is a collaboration between IPHCA and IQIN to achieve the goals below:

At Focus on Quality, community health centers work together to collectively strive for health equity through quality improvement focusing on a different priority population each month.
Monthly populations and topics were chosen based on a national calendar of health observances, clinical and operational quality metrics reported annually via UDS, and feedback from IPHCA’s Quality Advisory Committee.
Internal data reviews are used to find centers with best practices. Focus on Quality will also periodically discuss Health IT Innovations, elements of Value-Based Care, achieving Patient-Centered Medical Home (PCMH) recognition, and facets of the Chronic Care Model.

Focus on Quality is a monthly webinar series where Indiana’s community health centers work collectively to address the needs of the populations they serve every day.
Every month, Focus on Quality will highlight a specific population and feature content experts and identified tools, along with peer best practice sharing, to improve the health and well-being of that population.
Centers are encouraged to take these tools and best practices and implement them in their centers for quality improvement purposes supported by the IQIN team via technical assistance and evidence-based models for change.
